Understanding Key Insurance Terms
Copay
A copay is a fixed amount you pay for each therapy session, for example $20 or $40. This amount is determined by your insurance plan and is due at the time of service.
Coinsurance
Coinsurance is a percentage of the session cost that you're responsible for after your deductible is met. For example, if your plan covers 80%, you'll pay the remaining 20%.
Deductible
A deductible is the total amount you must pay out of pocket for covered services each year before your insurance starts covering costs. Until you meet your deductible, you're responsible for the full negotiated session fee.
Superbill
A superbill is a detailed receipt or invoice that you receive from your therapist when you pay out of pocket for services. It includes all the necessary information your insurance company needs to process an out-of-network reimbursement request.
A superbill typically includes:
- Your name and dates of service
- Your therapist's name, credentials, and National Provider Identifier (NPI)
- Diagnosis code(s)
- CPT (service) code(s)
- Total amount paid
- Provider's address and tax ID number
Reimbursement is not guaranteed and depends on your specific policy.
How We Handle Insurance Billing
Copays and Coinsurance
If your plan requires a copay or coinsurance, we will collect that payment at the time of your session.
Deductibles
If you have an active deductible:
- We will still submit your session claim to your insurance company.
- Once the claim is processed and applied to your deductible, we'll receive a breakdown from your insurance provider.
- We'll then issue you an adjustment invoice based on what you owe for that session.
- Your card on file will be charged within 48 hours of the invoice being issued.
If you'd like to use a different card or need time to plan for the charge, just let us know.
How to Check Your Insurance Benefits
To avoid surprises, we recommend calling your insurance company directly. Use the number on the back of your card and ask:
- Is Live Your Life Counseling (or my specific therapist) in-network?
- What is my copay or coinsurance per session?
- Do I have a deductible that I have to meet before therapy services are covered, and if so, how much of that deductible have I met?
- Is telehealth therapy covered?
- Do I need a referral or pre-authorization?
Out-of-Network Coverage
If we are not in-network with your insurance, you may still be able to use your out-of-network benefits. Here's how that works:
- You pay the full session fee up front.
- We provide you with a superbill (an itemized receipt).
- You submit the superbill to your insurance for partial reimbursement, based on your plan.
Note: Reimbursement is not guaranteed and may take several weeks.
Private Pay Option
Some clients prefer to pay privately for therapy. This option offers greater confidentiality (no diagnosis required by insurance), flexibility in treatment style and session frequency, and no limitations imposed by insurance companies.
Our private pay rates range from $125 to $160 per session. As a private pay client, you will be provided with a Good Faith Estimate of the expected cost of services before your first appointment.
